Addiction and Mental Health Services – Kingston, Frontenac, Lennox & Addington (AMHS-KFLA) encourages and welcomes feedback and complaints to support the ongoing delivery of quality services.
In receiving and responding to feedback and complaints, AMHS-KFLA recognizes and respects individuals’ human rights, including the rights to expression and to privacy.
There are several ways to provide feedback to AMHS-KFLA:

- In writing, using a printed form from AMHS-KFLA’s offices

- In person to any AMHS-KFLA staff member

All feedback is reviewed by our team who will forward relevant information to appropriate staff, departments or committees within three (3) business days. In order for AMHS-KFLA to address and resolve complaints, complainants (rather than those simply sharing concerns or negative feedback) are required to provide consent for use of their name in the complaint process and to provide a mechanism for contact, such as an email address or phone number. Every effort shall be made to handle complaints in a manner that respects the privacy and confidentiality of all involved. Anyone may request a digital or printed copy of the complete Feedback Policy using the same methods available for providing feedback. Accessible formats and communication supports are available upon request.
