A connected approach to health, housing and recovery

The Homelessness and Addiction Recovery Treatment (HART) Hub Kingston will be a coordinated, person-centred approach supporting people experiencing mental health challenges, substance use, and housing instability across the Kingston, Frontenac, Lennox and Addington (KFLA) region.

Rooted in the collaborative, low-barrier philosophy that built the Integrated Care Hub (ICH), the evolution to a HART model builds upon this work and will offer a system of integrated support in our community.

HART Hub Kingston will bring together four complementary service pillars: HART Connect, HART Reach, HART Haven and HART Homes.

Together, these services will create a connected pathway from immediate support and outreach through treatment, transitional housing and long-term supportive housing.

A place to begin. Immediate access, basic needs, health, social and addiction supports – the evolution of the ICH at 661 Montreal Street. Opened October 1, 2026

Care that travels. HART Reach will extend our HART Hub into communities across KFLA through mobile and community-based outreach to areas outside the Kingston urban core. HART Reach will bring support to people where they are, including our rural communities, shelters, and encampments, and will connect people to care, housing and other supports. Expected to launch in late 2026

A place to stabilize. HART Haven will offer short-term, transitional housing that will provide a safe place to stabilize, with treatment, recovery, and life-skills programming. Individuals will be supported with addiction and mental health needs and can access treatment as well as support to build the practical skills needed to move toward long-term housing and independence. Program development is currently in progress, and program timelines will be shared when confirmed.

A place to stay. Long-term, affordable, supportive housing for people who need ongoing, lower-intensity support to maintain housing stability, sustain recovery and remain connected to their community.
